Getting your Kentucky medical cannabis card is a new process, and Affinity Botanical Medicine is here to be your trusted guide. We’ve broken the process down into five straightforward steps to ensure you feel supported from your first click to your annual renewal.

Your Guided Path To Relief | A Kentucky Medical Cannabis Card

We are committed to transparent pricing and dedicated appointment times for every patient. Use the link above to request a day and time that works best for you. Once you submit your request, our office will call you within 24 hours to officially establish and confirm your appointment. Please Note: When requesting your appointment, be sure to subscribe to our new text reminders so you never miss an update about your visit!


Pricing & Fees:

  • Total New Patient Evaluation: $250
  • Scheduling Fee: A $50 scheduling fee is required when we call to confirm your time. This secures your slot and prevents No-Call/No-Show events. ***This is not an extra charge; it is applied directly to your total.
  • The remaining $200 is due at the time of your appointment.

Step 2: Prepare for Your Evaluation

To ensure your appointment is smooth and efficient, please gather the following items and bring them with you:


  • New Patient Forms: Please print, fill out, and sign the forms which are found under the Forms tab.
  • Medical Records: You only need to request records from your primary provider if you are seeking certification for cancer, epilepsy/seizure disorders, or multiple sclerosis (MS)/muscle spasticity. Dr. Bredenkamp can independently diagnose chronic pain, PTSD, and chronic nausea and cyclical vomiting syndrome during your visit.
  • A Valid Kentucky Photo ID: A Kentucky Driver's License or a Kentucky State ID Card to prove residency.

Step 3: Meet With Our Compassionate Physician

At your established time, you will have a private, confidential consultation with Dr. Bredenkamp. In this safe, comfortable space, he will review your medical history, discuss your health goals, and officially confirm or diagnose your qualifying condition.

Step 4: Receive Your Certification & Apply to the State of Kentucky

If approved, you will receive your official physician’s certification before you even leave the office. This document is valid for 60 days and is required to apply for your card.

Once your application is processed by the state, you will receive your official card and be ready to visit local KY dispensaries.

Step 5: Maintaining Your Certification

To maintain your legal status, Kentucky requires patients to be recertified annually. We make this simple: all recertification appointments can be conducted via a secure telemedicine (video) call from the comfort of your own home! Simply reach out to us 60 to 90 days before your card expires to request your renewal visit.


You must submit your recertification application to the State of Kentucky using the Patient and Caregiver Registry Portal no later than 30 days prior to the expiration date on your card.
