HOW IT WORKS

Getting your medical cannabis certification is a new process for everyone in Kentucky, and we are here to be your trusted guide. We’ve broken it down into five straightforward steps, ensuring you feel supported from your first click to your annual renewal.

Step 1: Understand the Fees & Secure Your Appointment

We are committed to transparent pricing and dedicated appointment times for every patient. Use the link below to navigate to our convenient, easy to use, secure scheduling portal to select a day & time that is best for you!  You can also reach out to us by phone or email to schedule your first appointment!


Scheduling Fee: $50

  • To confirm your appointment, we require a $50 scheduling fee.  We have this policy in place to ensure all patients who book an appointment are serious about their evaluation, which prevents No-Call/No-Show events that could take a slot from another patient in need.
  • This $50 fee is not an extra charge.  It is applied directly to the total cost of your appointment.
  • Once you pay the scheduling fee through our secure online link, your appointment will be officially confirmed in our calendar and we will reach out to you to answer any questions prior to your appointment.

New Patient Appointment Fee: $250

  • This covers the required in-person initial evaluation with Dr. Bredenkamp
  • The $50 scheduling fee is applied to this total.
  • The remaining $200 is due at the time of your appointment.

Step 2: Prepare for Your Evaluation

To ensure your appointment is smooth and efficient, please gather the following items. Having these ready and with you at your appointment is essential for our physician to complete your evaluation.

  • New Patient Forms: Please print, fill out, and sign the new patient forms which are found under the FORMS tab on our website.
  • Medical Records: Request a copy of your medical records from your provider IF you have any form of cancer regardless of stage, epilepsy or any other intractable seizure disorder, or Multiple sclerosis (MS), muscle spasms, or spasticity.  You will need records that clearly document your medical diagnosis in order to verify your qualifying condition and ensure compliance with state requirements. Dr. Bredenkamp can diagnose chronic or severe pain, chronic nausea or cyclical vomiting syndrome, and post-traumatic stress disorder
  • A Valid Kentucky Photo ID: This must be a Kentucky Driver's License or a Kentucky State ID Card to prove your residency.

Step 3: Meet With Our Compassionate Physician

At your scheduled time, you will have a private and confidential consultation with Dr. Bredenkamp. This is a safe space for you to have a comfortable conversation about your health. Dr. Bredenkamp will obtain a complete medical history, review your medical records if applicable, and discuss your condition in order to confirm or diagnose your qualifying medical condition.

Step 4: Receive Your Certification & Apply to the State of Kentucky

If you are approved, you will receive your official physician’s certification. This is the official document you need to apply for your medical cannabis card with the State of Kentucky using the Patient and Caregiver Registry Portal. The written certification is valid for 60 days.

Click on the Team Kentucky image to the right in order to open the Qualified Patient & Designated Caregiver Guide, which provides patients with the detailed, step-by-step instructions—complete with screenshots and direct links— provided by the Kentucky Medical Cannabis Program to walk you through the state’s online application process. Patient and Caregiver Registry Portal

Step 5: Maintaining Your Certification

To maintain your legal status and access to dispensaries, you must be recertified by a certifying physician at least annually, or more frequently as determined by the certifying physician on a case-by-case basis. You must submit your recertification application to the State of Kentucky using the Patient and Caregiver Registry Portal no later than 30 days prior to the expiration date on your card. Contact our office 60 to 90 days prior to your expiration date or request a follow up appointment using our secure patient scheduling portal.  For your convenience, all recertification appointments can be conducted via a simple and secure telemedicine (video) call from the comfort of your own home!
